Medical Day Health Centers
Our model of care is child focused and family oriented. Whether provided at one of our medical day health centers or in the home, services are coordinated around each child’s needs and support is offered to assist the family in assimilating their child’s health care into daily routines.
In addition to providing post-acute care in a traditional homelike environment, PHC provides nursing and therapy care to children who can be transported to the centers. The Medical Day Health Center allows easy access to skilled staff and better quality control.
Each center is staffed with pediatric trained professionals and equipped with state of the art medical equipment. Our centers provide intermittent and continuous nursing up to 12 hours per day as well as physical, occupational, speech, respiratory and developmental therapies.
The Medical Day Health Centers are certified by Medicaid as Pediatric Extended Care Centers to serve children with complex medical conditions and special needs.
The professional teams at the Medical Day Health Centers consist of the following:
- Skilled neonatal and pediatric RNs and LPNs
- Licensed occupational, physical and speech therapists
- Child developmental specialists
- Child life specialists and early interventionists
- Social workers
- Nutritionists
- Certified nursing assistants and medical technicians
PHC’s multidisciplinary team works with the child's physician and family to develop a plan of care that focuses on goals such as:
- Optimizing quality of care to avoid hospitalizations and visits to the emergency room
- Increasing peer interaction and developmental stimulation
- Improving parents’ coping skills
PHC’s professional team monitors and detects subtle changes in a child's status, communicates these changes to the physician and provides early intervention treatment.
