Pediatric Health Choice provides a seamless transition from the hospital to the home or home like environment or to one of PHC’s Medical Day Health Centers.

For the children who are homebound after hospital discharge, PHC’s home care professionals provide a broad array of home care services ranging from infusion, wound and burn care to ventilator assistance.

PHC can help prevent costly re-hospitalization and repeat visits to the emergency room.  PHC provides private duty shift nursing and skilled intermittent visits.  We provide training and education to the caregiver and/or family.  Our 24-hour-a-day, 7-day-a-week clinical support system assures optimum care for the children and guidance to the families.
