Pediatric Health Choice’s customer service representatives can help the child’s family and providers obtain insurance authorization for the reimbursement of the prescribed care and assist in making other payment arrangement when needed.

Our staff coordinate the documentation required by Medicaid, various government funding programs and private insurance companies to ensure timely approval for the delivery of care and services.

Pediatric Health Choice is a provider for Medicaid, Children Medical Services, Developmental Waiver, Blue Cross Blue Shield, Aetna, Amerigroup, TriCare and many others.  Our insurance guides lists all government programs, insurance companies and networks that PHC participates in and the services PHC can provide under contract.

• Complete the appropriate PHC Referral form and/or physician's orders/ Letter of Medical Necessity form and fax back with physician’s signature
